Medical Examinations for NJ Crossing Guards

School Crossing Guard Physical Form (Word) — The New Jersey Safe Routes to School Resource Center’s Crossing Guard Training and Resources program has developed a model crossing guard job description and a model medical examination form. These examples provide a full description of the physical requirements of the crossing guard position. The job description is intended to assist job applicants in determining if they are suited to the position, and to assist supervising officers in discussing qualifications with prospective employees. The medical examination form can be used by doctors to determine an employee’s fitness for crossing guard duties.
